Activating Healthcare Assets for Housing & Health Equity

This virtual event was hosted via Zoom on June 7, 2022 and  brought together a panel of healthcare leaders to discuss how recent challenges are impacting our work related to social determinants of health.

The Center for Community Investment, GMHF’s national partner, and a panel of Minnesota based healthcare institutions shared how they are using a wide range of assets to partner and address affordable housing in their communities.

Panelists shared project examples, emerging housing and health partnership opportunities, and when community partners are needed to achieve healthcare mission objectives. Scroll down to see speakers and the event agenda.

Host Speakers

Lucas Nesse — Minnesota Council of Health Plans

President and CEO

Lucas joined the Minnesota Council of Health Plans as its President and CEO in 2019. In this role, Lucas works with the state’s non-profit health plans to pursue high quality and affordable health services for Minnesotans. Prior to assuming this role Lucas was the director of health policy for the Minnesota Business Partnership, a large employer membership organization. Lucas earned a Bachelor of Arts in Political Science from Gustavus Adolphus College and his Juris Doctor from William Mitchell College of Law.

Warren Hanson — Greater Minnesota Housing Fund

Founding President and CEO

Warren is the President & CEO as well as founder of Greater Minnesota Housing Fund and Minnesota Equity Fund. Warren leads the executive team and works closely with state and local governments, philanthropy, investors, and community leaders to assemble resources and design strategies to meet affordable housing needs in Minnesota. He brings over 35 years of development finance experience to GMHF.

Eric Muschler — Greater Minnesota Housing Fund

Director of Housing & Health Equity

As Director of Housing & Health Equity, Eric works to explore the multiple connections and benefits of housing with social determinants of health. Eric has experience working for 15 years, toward driving and expanding economic opportunity for people of color and low-income communities. Eric believes that housing stability is the foundation for opportunity especially when connected with other economic supports like education, workforce, asset and wealth building, and community economic development.

Robin Hacke — Center for Community Investment (CCI)

Executive Director and Co-Founder

Robin and CCI are dedicated to overcoming disinvestment so that everyone has a fair chance to lead a healthy and productive life. CCI has developed a framework that leading health systems have used to accelerate investments in affordable housing in their communities.

Antony Stately — Native American Community Clinic

President and CEO

Antony is the Executive Officer and President for the Native American Community Clinic in south Minneapolis, which provides primary care, dental care, and behavioral health services the Twin Cities Native American community. He has taught in clinical graduate programs in Wisconsin, Seattle, and Los Angeles and has served as a consultant and advisor to the CDC, HRSA, SAMHSA, the Native American AIDS Prevention Center (NAAPC), the US-Mexico Border Health Association (PAHO/WHO), and numerous NGOs and non-profits delivering health services to tribal and indigenous communities nationally and internationally.

John Swanholm — M Health Fairview

VP Community Advancement & President, Fairview Foundation

John serves as Vice President, Community Advancement for M Health Fairview, and President, Fairview Foundation.

He provides executive leadership for Community Advancement and the Center for Community Health Equity, comprised of areas where there is significant synergy for M Health Fairview, including: Community Health Equity & Engagement; Community Health Improvement operations and programs; Community Benefit & Measurement; and Philanthropy/Foundation. He also leads organizational imperatives around Anchor Institution Mission and Community & Clinical integration.

John and his team acknowledge that optimal health begins where we live, work, learn, worship, and play. He leads the organization’s focus on innovative and upstream approaches to create healthier and more vibrant communities.  The team is focused on bridging and leveraging M Health Fairview’s assets and strengths with community assets and strengths to drive a healthier future.

Andy McMahon — United Health Group

VP of Community Engagement and Investment

Andy is an entrepreneurial professional with more than two decades of experience in the fields of affordable housing, health care, human services, and the integration among them. Andy has a proven record of cultivating and executing collaborations across government agencies, philanthropy, healthcare entities, and community partners. Andy has strong and diverse government relations experience in affordable housing, health care, and criminal justice sectors; and has a time-tested commitment and successful track record integrating public systems and private partners t better serve individuals and families with complex health needs.

Bukata Hayes — Blue Cross and Blue Shield of Minnesota

VP of Racial & Health Equity

Bukata works with Blue Cross and Blue Shield of Minnesota (Blue Cross) to identify and coordinate specific steps which can be taken to ensure that a racial and health equity lens is applied to every aspect of Blue Cross’ work and culture. He serves as an advisor, bridge builder, strategic leader and partner, to identify and execute initiatives that can bring about greater inclusiveness, representation, and lasting and meaningful change.

  • The Native America Community Clinic — Antony Stately, President and CEO will discuss plans to build a new expanded community clinic in the Native American Cultural Corridor, with 94 units of affordable housing and possible respite care beds for the clinic.
  • M Health Fairview — John Swanholm, Vice President, Community Advancement & President, Fairview Foundation will discuss the creation of a Community Center for Health Equity, reuse of property/assets to address SDOH and assets set aside for affordable housing.
  • United Health Group — Andy McMahon, Vice President of Community Engagement and Investment will discuss UHG’s major housing investments in the Minnesota communities of Duluth and Minneapolis, executed in partnership with Common Bond Communities, Center City Housing Corporation and Greater Minnesota Housing Fund.
