Annual Multifamily Long-Term Loan Reporting 2018

Property managers with GMHF long-term loan funding are asked to submit the GMHF Reporting Workbook by April 30, 2019.

Greater Minnesota Housing Fund (GMHF) annually collects demographic and financial data for each property in its long-term loan portfolio.



For each property under your management with GMHF long-term loan funding, please submit the following to by Tuesday, April 30, 2019.

Financial reports should cover fiscal year 2017 & 2018, and demographic data should be as of December 31, 2018.

GMHF Reporting Workbook: Download the Reporting Workbook. Also download the Financial Tab. Following the instructions on tab 1, complete tabs 2, 3, and 4. Submit the workbook as an Excel file, not as a PDF or print-out. Please note: Per your executed loan documents – GMHF requires notice and approval of any sale, transfer, or conveyance of any portion of the owner’s interest in properties with GMHF financing. This includes a change to the General Partner. If the owner has changed and GMHF’s loan documents have not been amended to reflect the change, the owner must select “Yes” to answer question I.4 of the self-certification form.

**New** Financial Statements Tab. This year, GMHF is requiring that the last two (2) years of financials to be filled out completely in Tab 5. Please also forward the last two (2) year of audited financials.  If audited financials are not available, GMHF will accept unaudited financial statements. Attach these reports as a PDF along with a completed copy of this Excel workbook to See the instructions tab of the Reporting Workbook for more detailed information.

Insurance Certificate: Please provide a copy of the current property and liability insurance certificates for the properties below. As stated in the self-certification, the policies must be endorsed with a standard mortgagee clause with loss payable to Greater Minnesota Housing Fund and show Greater Minnesota Housing Fund as an additional insured.
